Understanding ADHD

Okay, 'executive functions' is one of those terms I hear all the time. What is that in… like, normal human language? Is that why I can't start doing my taxes even though I know I have to?

Exactly. That's a perfect example. Executive functions are things like planning—so, figuring out the steps to do your taxes. Prioritizing—deciding to do taxes instead of reorganizing your bookshelf. And task initiation—which is the huge one—that’s the physical act of just *starting* the thing.

Beau

Task initiation. The wall. I call it 'The Wall.' It feels like a physical barrier between me and the thing I need to do.

That's a great way to put it. And it's not just about starting tasks, it's also about impulse control. That's another executive function. The prefrontal cortex is supposed to be the brake pedal. For an ADHD brain, that brake is a little... less reliable.

Which explains why I'll suddenly buy a banjo online at two in the morning. The impulse just hits and there's... no stop sign.

Right. And there’s a chemical reason for that, too. It’s tied to dopamine. The brain's reward system, which is regulated by a region called the basal ganglia, runs on dopamine. In ADHD brains, the dopamine system is less efficient.

So… my brain is constantly looking for things that will give it a little dopamine hit? Like buying a banjo, or… oh, chasing a new, exciting idea instead of finishing the report that's due tomorrow.

You've got it. The report is boring. It offers a delayed, small reward. The new idea is novel and exciting—it offers an immediate (or promised) hit of dopamine. Your brain, being starved for it, will always gravitate toward the more stimulating option. That's the core of the inattention symptom. It's not that you *can't* pay attention; it's that it's incredibly difficult to direct your attention to things that aren't inherently interesting.

Okay, that makes so much more sense than just 'being distractible.' But what about the emotional side? Because it's not just about focus. Sometimes I feel like my emotions have no volume knob. It's either off, or it's at eleven.

Jo

That's a huge, often overlooked part of ADHD. It's called emotional dysregulation. And there's a brain region for that, too: the anterior cingulate cortex, or ACC. You can think of it as the bridge between your emotional brain—the limbic system—and your thinking brain, that prefrontal cortex we talked about.

A bridge… so what, my bridge has a toll booth that's always closed?

Jo

More like the bridge has trouble managing traffic flow. The ACC helps you shift attention, regulate emotional responses, and notice your own errors. In an ADHD brain, that connection is weaker. So an emotion, like frustration or excitement, can flood the system without the 'thinking' brain being able to step in and say, 'Hey, let's calm down, this isn't that big a deal.'

So someone cuts me off in traffic. My first reaction is just pure rage, and it takes me way longer than it should to get back to a state of... not rage.

Jo

Precisely. The emotional impulse is strong, and the mechanism to regulate it is weak. That's the dysregulation. It's not that you're an angry person, it's that your brain's braking system for emotions isn't as effective.

Beau

So, just to recap my brain here: I've got an underpowered CEO in the prefrontal cortex, a reward system that’s a dopamine fiend, and a faulty bridge between my thinking and feeling parts.

That's a pretty good summary, actually. It sounds kind of bleak, but understanding the 'why'—the neurobiology—is the absolute first step. It lets you stop blaming yourself for having a 'broken' brain and start thinking about how to build supports for the brain you actually have.
