Okay so — picture this. It's like, three in the afternoon. You've been sitting at your desk since what feels like the beginning of time, you've got that specific kind of tired that isn't really sleepy, it's more like... foggy. And someone — maybe it's you, let's be honest, it's you — walks to the kitchen or the vending machine or wherever, and you grab a cold can of regular Coke. You crack it open, you take that first sip, and for a moment, everything feels fine. And I'm telling you this because — and I recognize this is a slightly strange way to open a podcast that is nominally about weight loss and metabolic health — but that can of Coke? That might be the most useful thing we talk about today.

It really might be. And I know that sounds like a bit of a stretch when you first hear it, but — no, genuinely, if I could pick one single everyday moment to use as a kind of... window into how the body handles energy, that'd probably be it. There's so much happening in the fifteen or twenty minutes after you drink that can that most people never think about. And once you see it, you kind of can't unsee it.

Right, and I think that's kind of the whole ethos of what we're doing here, actually. Like, we're not going to come at this with a list of things you should or shouldn't eat. We're going to try to just — explain what's actually going on. Because I feel like most of us have spent years being given instructions about food and weight without anyone ever taking five minutes to explain the underlying... machinery? Is that the right word?

Machinery works. Yeah. And honestly, in my experience sitting across from patients — people who have been managing their weight or their blood sugar for years — one of the most common things I hear is some version of 'nobody ever explained it to me like that.' Like they've been following rules without ever being handed the rulebook's explanation. And that gap matters. It matters a lot.

Okay so before we get back to the Coke — and we are coming back to it, that's a promise — I want to back up and ask what is honestly kind of an embarrassingly basic question, which is: what is food actually doing when it goes into your body? Like mechanically. What is the body treating it as?

No, it's not embarrassing at all — it's exactly the right place to start. So the simplest frame is this: food is fuel. But — and this is important — it's not one kind of fuel. It's more like three different types that the body uses in different ways, at different speeds, for different purposes. And those three types are what you've probably heard called macronutrients: carbohydrates, proteins, and fats.

Okay and I feel like everyone's heard those words their whole lives and also kind of... doesn't totally know what they mean? Like I know carbs are a thing and protein is a thing but I don't think I could've told you until fairly recently what the body is actually reaching for each one for.

That's completely fair. So here's how I think about it — carbohydrates are the body's quick-access energy. They break down relatively fast into glucose, which is just a form of sugar, and glucose is basically the body's preferred, most immediately usable fuel. Your brain runs on it, your muscles use it. It's the fast-burn option. Fats, on the other hand, are the opposite — they're denser, slower to access, but really long-lasting. And protein is kind of in its own category — its primary job isn't energy at all. It's more like the body's construction material. Building and repairing things. Muscle tissue, enzymes, all of that.

So it's almost like — okay, I'm going to try an analogy here and you can tell me if it falls apart — but it's like if you think of a kitchen. Carbs are the stuff in the front of the fridge, the things you grab when you need to put dinner on the table in fifteen minutes. Fats are more like the stuff in the back of the freezer, the slow-cook Sunday roast. And protein is... the shelving unit holding the whole kitchen together?

Ha — okay, I actually really like the first two. The freezer one especially, that's genuinely a good way to think about fat as an energy reserve. The protein-as-shelving is a little — let me just gently adjust that — protein is less about structure of the kitchen and more like the kitchen's repair crew. Like if something breaks, protein is what fixes it. But yes, carbs as the fast-access, grab-it-now fuel and fat as the slow-burning reserve — that's exactly right.

The repair crew. Okay, that's better, yeah. So we've got fast fuel, reserve fuel, and the repair crew. But here's the thing — not all carbs are the same, right? Like I feel like this is where it starts to get a bit more interesting, because I've definitely heard the phrase 'slow-release carbs' and sort of vaguely nodded along without really knowing what that meant.

Right, so this is actually a really important distinction and I think it's where a lot of the confusion about carbs comes from. When people say 'carbs are bad,' what they're often — without realizing it — reacting to is a very specific type of carbohydrate, which is the simple, fast-release kind. But there's a whole other category, complex carbohydrates, that behave totally differently in the body.

Okay so what's the actual difference? Like if I eat a bowl of oats versus — I don't know — a spoonful of white sugar, what's happening differently?

Jo

So oats, wholegrain bread, legumes — those are complex carbohydrates. They've got this longer, more intricate molecular structure, and that means your digestive system has to do more work to break them down. It takes time. So glucose — the actual fuel — gets released into your bloodstream gradually, in a kind of steady trickle. Your body has time to manage it. Versus simple sugars, like the white sugar in your example, or the high-fructose corn syrup in — well, in a can of Coke — those are already basically pre-broken-down. They need almost no digestion at all. So they hit the bloodstream almost immediately. It's like the difference between a slow drip and... just upending a bucket.

Oh, the bucket is a great image. And so — is it right to say the body is essentially getting information at different speeds? Like, with the oats it's getting a kind of slow, manageable signal, and with the Coke it's suddenly getting this — urgent message?

Jo

That's actually a really nice way to frame it. Yes — the body has to respond to what it receives, and the speed of that signal matters enormously for how the response plays out. And I want to just quickly note — because I know 'carbs are bad' is such a dominant message out there — that this distinction is exactly why that framing is so unhelpful. It's not that carbohydrates are bad. It's that different carbohydrates ask very different things of your body. Understanding that is so much more useful than just... blanket avoidance.

Okay, so — let's go back to the can. You crack it open, three in the afternoon. Standard can of Coke, right? And I looked this up because I wanted to be precise about it — there's about 39 grams of sugar in a standard can. Which, when you say it out loud, is — that's a lot, right?

It is a lot. And what's particularly significant is that almost all of it is in the form of simple sugars — specifically high-fructose corn syrup or sucrose depending on the formulation, but either way, sugars that require virtually no breakdown before they enter the bloodstream. So you take that first sip, and within minutes — not hours, minutes — you have a substantial wave of glucose entering your blood. That's what a blood sugar spike actually is. It's not metaphorical, it's literally glucose flooding into the bloodstream faster than the body's normal management systems can pace it.

Okay and — and here's the question I think a lot of people might be sitting with right now, which is: so what? Like, what's the actual problem with glucose just... being there? In your blood?

Jo

Great question, and it gets to the heart of why this matters. So glucose in the bloodstream isn't inherently bad — you need it, it's your fuel. But the concentration of it matters. You need it within a certain range. Too much glucose sitting in the blood for too long can start to damage things — blood vessel walls, nerves — and the body knows this. So it has a very finely tuned response system. And the center of that response system is an organ called the pancreas.

The pancreas. Which I feel like most people know exists but couldn't tell you exactly what it does.

Jo

Totally. So the pancreas is — it has a few jobs, but the one we care about right now is that it's essentially the body's blood sugar sensor and response system. It's monitoring glucose levels in the blood more or less constantly. And when it detects that blood sugar is rising — which it does very quickly after you drink that Coke — it releases a hormone called insulin. And insulin's job is to open the doors of cells so that glucose can get in — to be used for energy right away or stored for later.

So insulin is like — okay, insulin is like a key. It's the thing that unlocks the cell so glucose can actually get inside and be used.

Jo

Exactly, that's the classic analogy and it's a good one. Or — I sometimes think of it as a kind of logistics coordinator. Like, suddenly there's this enormous shipment of packages arriving — all this glucose — and insulin is the one routing it, saying, okay, some of this goes here to be used immediately, some of this gets stored in the liver, some in muscle tissue. It's doing this rapid, coordinated dispatch operation. But here's the catch — when the spike is very large and very fast, like after a can of Coke, the pancreas releases a correspondingly large surge of insulin. And that big insulin response is so effective that it can actually pull blood sugar back down very sharply. Further than you need it to. And that's the crash.

Wait — so the crash — the thing where you feel terrible an hour after the sugar hit — that's not the sugar wearing off. That's the insulin doing its job... maybe too well?

Jo

That's exactly it. The insulin overshoots. It's doing precisely what it's designed to do, it's just that the signal was so large that the response was large, and now your blood sugar is lower than it was before you drank the Coke. And your body registers that as an energy dip — you feel foggy, tired, sometimes a bit irritable. And the insidious thing is that your body then starts looking for another quick hit of glucose to bring things back up. Which is why you might find yourself reaching for something sweet again not that long after.

That is — okay, that is one of those moments where I feel like things genuinely click into place. Because I have absolutely been in that cycle and just thought of it as like, I have no self-control around snacks at 4pm. And it's like — no, there's an actual physiological thing happening.

Right, and this is something I feel quite strongly about because the way we talk about this kind of eating behavior is so often framed as a willpower issue, a character issue. And it's just — it isn't. What you're describing is a completely predictable physiological response to a blood sugar roller coaster. And a lot of people are riding that roller coaster multiple times a day, with every sugary drink, every fast-digesting snack, without having any idea that there's a mechanism underneath it.

So I want to just — slow down for a second here, because I think we should sit with what we've just explained. We just described, in fairly concrete terms, why you feel terrible after a 3pm soda. And like — that's something most people have felt dozens or maybe hundreds of times. Possibly without ever knowing the name of the thing that was happening to them.

And I think that's worth pausing on, yeah. Because there's something important in recognizing that what we just described is the body working exactly as it was designed to work. Nothing is broken here. The system is actually — it's quite elegant, when you see it clearly. The pancreas is doing its job. The insulin is doing its job. The problem isn't the system. It's that the system was built for a world where this kind of sugar hit was a rare, occasional event. Not something that happens three, four, five times a day.

So it's less that Coke is evil — I mean, I'm not trying to rehabilitate Coke here — but it's more that... understanding what it asks of your body changes how you think about it. Like it's not about moral judgment, it's about seeing the mechanism.

Exactly. And when this pattern happens occasionally, the body handles it fine. It's remarkably resilient. But when it's happening constantly — when the blood sugar is spiking and crashing repeatedly, day after day — something starts to shift over time. The system starts to strain. And that's — well, that's what the next episode is going to be about. Because those long-term consequences are important to understand, and they're also, I'd argue, just as fascinating in their own way.
