What Is a Traumatic Brain Injury?

A traumatic brain injury, or TBI, happens when a sudden, external physical force damages the brain. Think of the brain like a soft, delicate command center protected by the skull. Most of the time, this hard casing does its job. But a forceful bump, blow, jolt, or penetrating wound can cause the brain to move, twist, or be pierced, leading to injury.

This damage isn't just a bruise. It can disrupt the brain's normal function, affecting everything from memory and emotion to movement and sensation. The effects can be temporary or long-lasting, depending on what part of the brain is hurt and how badly.

Types of Brain Injuries

TBIs are generally sorted into two main categories: closed and penetrating. The difference is simple: did something break through the skull or not?

A closed brain injury is the more common type. Here, the skull remains intact, but the brain is damaged from being shaken violently. This can happen in a car crash, a fall, or a sports collision. The impact can cause the brain to slam against the inside of the skull, leading to bruising, swelling, or tearing of nerve fibers. A concussion is a common example of a mild closed TBI.

A penetrating brain injury occurs when an object, like a bullet or a sharp piece of debris, breaks through the skull and enters the brain tissue. These injuries are often more localized, damaging the specific area where the object entered. They are typically very severe.

Common Causes

TBIs can happen to anyone, anywhere. However, some situations and activities carry a higher risk. Understanding the common causes can help highlight the importance of safety measures like wearing helmets and seatbelts.

Measuring Severity

Not all brain injuries are the same. To help understand the extent of the damage and predict outcomes, healthcare providers classify TBIs by their severity: mild, moderate, or severe.

This classification is often made in an emergency setting using tools like the Glasgow Coma Scale (GCS). This scale assesses a person's ability to open their eyes, speak, and move. The score helps determine the initial level of injury.

A key factor in determining severity is the person's state of consciousness. How long were they unconscious, if at all? Do they have memory loss surrounding the event?

Mild TBI: A brief change in mental status or consciousness (like being dazed or confused) or loss of consciousness for less than 30 minutes. Most concussions fall into this category.

Moderate TBI: Loss of consciousness lasting from 30 minutes to 24 hours.

Severe TBI: Loss of consciousness for more than 24 hours.
