You've moved past simply identifying which foods have carbohydrates. The next step is precision. While basic carb counting focuses on consistency—eating a similar amount of carbs at each meal—Advanced Carbohydrate Counting (ACC) is about flexibility. It's a method used with intensive insulin therapy, like an insulin pump or multiple daily injections, to precisely match your insulin dose to your food.
ACC lets you adjust your insulin based on what you actually eat, moment by moment. It gives you the freedom to eat different amounts of carbs at different meals, because you have the tools to calculate the exact insulin dose needed each time. This requires two key personal numbers: your Insulin-to-Carbohydrate Ratio (ICR) and your Insulin Sensitivity Factor (ISF).
Your Personal Ratios
Your Insulin-to-Carbohydrate Ratio (ICR) tells you how many grams of carbohydrates are covered by one unit of rapid-acting insulin. For example, an ICR of 1:10 means one unit of insulin covers 10 grams of carbs.
This ratio isn't a universal constant; it’s unique to you and can even change throughout the day or due to factors like exercise and stress. A common starting point is the 'Rule of 500'. To find your estimated ICR, divide 500 by your total daily dose (TDD) of insulin (including both long-acting and rapid-acting).
Next is your Insulin Sensitivity Factor (ISF), also known as a correction factor. This number tells you how much your blood glucose (BG) will drop, in mg/dL, for every one unit of rapid-acting insulin. It’s used to correct a high blood glucose reading.
A common way to estimate the ISF is the '1800 Rule' for rapid-acting insulin.
Remember, the 'Rule of 500' and '1800 Rule' provide estimates. Your actual ICR and ISF should be determined and fine-tuned with your healthcare provider.
Calculating a Mealtime Bolus
With your ICR and ISF, you can calculate a precise mealtime bolus. The calculation has two parts: a food bolus to cover the carbs you're about to eat, and a correction bolus to bring a high blood glucose level back to your target range.
First, calculate the food bolus by dividing the total grams of carbs in your meal by your ICR.
Next, if your current blood glucose is above your target, calculate the correction bolus. Subtract your target BG from your current BG, then divide the result by your ISF.
Finally, add the two parts together for your total mealtime dose.
The Nuance of Net Carbs
Counting every single carbohydrate isn't always accurate. Some carbs, like fiber and certain sugar alcohols, don't impact blood glucose in the same way as starches and sugars. Dosing insulin for them can lead to hypoglycemia. This is where calculating 'net carbs' becomes important.
For dietary fiber, since it's not digested, you can subtract its full gram amount from the total carbohydrates. If a food has 20g of total carbs and 5g of fiber, you would dose for 15g of carbs.
When to subtract fiber: Only subtract fiber if the food contains more than 5 grams of it. For smaller amounts, the impact is usually negligible.
Sugar alcohols (like xylitol, sorbitol, and mannitol) are a bit different. They are partially absorbed by the body, so they can raise blood glucose slightly. A general rule is to subtract half of their gram amount from the total carbs.
For example, if a protein bar has 25g of total carbs, 6g of fiber, and 10g of sugar alcohols, the calculation looks like this:
