LMHR Diagnostic Criteria

The LMHR Triad

You’ve likely seen a standard lipid panel before. It's a snapshot of the fats, or lipids, in your blood. Doctors look at markers like LDL cholesterol, HDL cholesterol, and triglycerides to assess metabolic health. But a specific and striking pattern can emerge for certain individuals who adopt a low-carbohydrate or ketogenic diet. This isn't just a slight shift in numbers; it's a dramatic change that constitutes a unique metabolic profile known as the Lean Mass Hyper-Responder, or LMHR phenotype.

To be classified as an LMHR, your blood work must meet a specific triad of criteria simultaneously. It isn't a sliding scale; you either meet all three thresholds or you don't. The numbers must be measured while you are following a carbohydrate-restricted diet, as this is the trigger for the phenotype.

The LMHR Triad: • LDL Cholesterol: ≥ 200 mg/dL • HDL Cholesterol: ≥ 80 mg/dL • Triglycerides: ≤ 70 mg/dL

Notice the combination. While the LDL-C is very high, both the HDL-C (often called 'good' cholesterol) and triglycerides are in exceptionally optimal ranges. This isn't the typical lipid profile seen in metabolic syndrome, where high LDL is often accompanied by low HDL and high triglycerides. In LMHR, the pattern is inverted, with high LDL, high HDL, and low triglycerides. The typical person exhibiting this profile is lean, fit, and metabolically healthy by most other standards.

Reading Your Lipid Panel

When you get a lipid panel from a lab, you'll see a report with several values. To check if you match the LMHR phenotype, you need to find the three key markers. These are usually clearly labeled as LDL-C, HDL-C, and Triglycerides. Ignore any other numbers for this specific purpose, like total cholesterol or VLDL. Simply compare your numbers for these three markers against the triad's thresholds.

It’s important to note that lab results may present LDL-C as either a direct measurement (LDL-d) or a calculated value (LDL-c). For most standard panels, you'll be looking at the calculated number. This is sufficient for identifying the phenotype.

Scientific research often needs to cast a slightly wider net to capture enough participants for a study. Because of this, you may see slightly different criteria used in clinical trials. For example, a recent study looking at heart scans in ketogenic dieters used what could be called 'Near-LMHR' criteria. This allowed them to include individuals who were very close to the classic definition.

Near-LMHR Research Criteria: • LDL Cholesterol: ≥ 190 mg/dL • HDL Cholesterol: ≥ 60 mg/dL • Triglycerides: ≤ 80 mg/dL

These research-grade numbers are useful for scientific inquiry, but the stricter triad (200/80/70) remains the community-recognized standard for identifying as a classic Lean Mass Hyper-Responder from a self-experimentation standpoint. If your numbers fall into this 'near' category, it suggests you are on the cusp of the phenotype, but not quite a textbook case. The key is that the pattern—very high LDL, very high HDL, and very low triglycerides—is the defining feature.
