Measuring Disease
To understand health trends, we first need to count. Medical statistics give us two main ways to measure how a disease affects a population: incidence and prevalence. They sound similar, but they tell very different stories.
Incidence

Prevalence

The total number of cases of a disease existing in a specific population at a particular point in time.
Think of it like a bathtub. Incidence is the rate at which water pours in from the faucet—it measures only the new water. Prevalence is the total amount of water in the tub at a specific moment. It includes both new and old water.
So, a disease like the common cold has a high incidence (many new cases all the time) but a low prevalence (people recover quickly, so not many have it at any one moment). A chronic condition like diabetes has a lower incidence, but a high prevalence because people live with it for a long time.
Risk and Relationships
Once we can measure disease, we want to understand what causes it. This involves looking at risks and relationships between different factors, like lifestyle and health outcomes. But we have to be careful not to jump to conclusions.
A key concept here is the difference between correlation and causation. Correlation means two things happen together. Causation means one thing causes the other to happen.
For example, ice cream sales and shark attacks are correlated. They both increase in the summer. But eating ice cream doesn't cause shark attacks. The hidden factor, or confounding variable, is the warm weather that leads to more swimming and more ice cream consumption.
In medicine, finding a correlation is the first step. For instance, researchers might notice that people who drink coffee have lower rates of a certain disease. That's a correlation. But does coffee cause this effect? Or do coffee drinkers share other habits—like exercising more or smoking less—that are actually responsible? To figure that out, researchers need to design specific studies.
How We Study Health
Medical research relies on different types of studies to investigate these relationships. They generally fall into two broad categories: observational studies and experimental studies.
In an observational study, researchers watch a group of people and measure outcomes without intervening. They are simply observing what happens naturally.
Two common types are cohort and case-control studies.

Observational studies are great for identifying potential links and are often used when it would be unethical to conduct an experiment, like asking people to start smoking.
For more definitive proof of causation, researchers use experimental studies, the most common of which is the Randomized Controlled Trial (RCT).
In an RCT, participants are randomly assigned to one of two groups: one receives the treatment being tested (like a new drug), and the other receives a placebo or standard treatment (the control group). By randomly assigning people, researchers can minimize the effect of other factors and be more confident that any differences in outcome are due to the treatment itself.
Because they provide strong evidence for cause and effect, RCTs are often called the "gold standard" of medical research. They are crucial for testing the safety and effectiveness of new medicines and therapies.
