What Are Clinical Trials?
Clinical trials are research studies performed with people to evaluate a new medical treatment, intervention, or device. The goal is to determine if the new approach is safe and effective for humans. A Brief History
The idea of testing treatments isn't new. One of the earliest recorded clinical trials was conducted in 1747 by James Lind, a ship's surgeon. He wanted to find a cure for scurvy, a disease that was devastating sailors. He took 12 sailors with scurvy and divided them into pairs, giving each pair a different daily treatment. The options included cider, sulfuric acid, vinegar, sea water, and two oranges and a lemon.
The sailors who ate the citrus fruits recovered quickly. Lind's simple but systematic experiment demonstrated that citrus could prevent scurvy. This laid the groundwork for the controlled, scientific approach used in modern clinical trials.
Over the centuries, this process has become much more sophisticated and regulated. Today, trials follow strict scientific standards and ethical guidelines to protect the participants while generating reliable data.
The Four Phases
Modern clinical trials are typically conducted in a series of steps called phases. Each phase is designed to answer a different research question. If a treatment is successful in one phase, it can move on to the next.
Phase I: The first step in testing on humans. A small group of healthy volunteers (usually 20-80) receive the treatment. The primary goal is to assess its safety, determine a safe dosage range, and identify side effects.
Phase II: If the treatment is deemed safe, it moves to Phase II. This phase involves a larger group of people (100-300) who have the condition the treatment is intended for. The focus shifts to effectiveness: Does it work? This phase also continues to evaluate its safety.
Phase III: This is a large-scale test. The treatment is given to hundreds or thousands of patients (1,000-3,000+). These trials compare the new treatment to existing standard treatments or a placebo. The goal is to confirm its effectiveness, monitor side effects, and collect information that will allow it to be used safely. Successful Phase III trials are usually the final step before a drug company applies for approval from regulatory agencies like the U.S. Food and Drug Administration (FDA).
Phase IV: These are post-marketing studies, conducted after a treatment is approved and available to the public. These trials gather more information on the drug's long-term risks, benefits, and optimal use in the general population.
