When the Pump Falters
Congestive heart failure (CHF) doesn't mean the heart has stopped working. Instead, it means the heart muscle has become too weak or stiff to pump blood effectively throughout the body. Think of it like a sump pump in a basement that can't keep up with incoming water. The water level rises and starts to back up. In CHF, the "water" is blood, and it backs up into the lungs and other parts of the body.
This backup is what causes the "congestive" part of the name. When the left side of the heart struggles, blood gets backed up in the vessels of the lungs. Fluid leaks into the lung tissue itself, a condition called pulmonary edema. This makes it difficult to breathe, especially when lying down or during physical activity.
If the right side of the heart is weak, blood backs up in the veins returning from the rest of the body. This pressure forces fluid out of the blood vessels and into surrounding tissues. The result is swelling, or edema, most commonly seen in the legs, ankles, and feet. In some cases, fluid can also accumulate in the abdomen.
Why Hearts Weaken
Heart failure is often the end result of other conditions that have damaged or overworked the heart muscle over time. One of the most common culprits is coronary artery disease (CAD). In CAD, the arteries supplying blood to the heart muscle become narrowed by plaque buildup, a process called atherosclerosis. This reduces blood flow, starving the heart muscle of oxygen and nutrients, which can weaken it permanently.
Long-standing high blood pressure, or hypertension, is another major cause. Uncontrolled high blood pressure forces the heart to work much harder to circulate blood. To cope with this heavy workload, the heart muscle thickens, just like any other muscle would. Over time, this thickened muscle can become stiff and less effective at pumping, eventually leading to heart failure.
Problems with the heart's valves can also lead to failure. Heart valves act like one-way doors, ensuring blood flows in the correct direction. If a valve is narrowed (stenosis), the heart has to pump harder to push blood through it. If a valve leaks (regurgitation), blood flows backward, forcing the heart to pump the same blood over again. Both scenarios put a chronic strain on the heart that can weaken it.
Recognizing the Signs
The symptoms of heart failure stem directly from the fluid congestion and the body's reduced blood supply. They often develop gradually, but can also appear suddenly.
